Union signature: if the sponsor of the programme is part of a union, the union must also sign an agreement. Sponsor of the program: name of the company or union that sponsor the apprentice, recognizes the rights and duties: the apprentice and the sponsor must be originally where it is indicated. The apprentice undertakes to do the work and take the corresponding courses. Aggregate statistical information: The apprentice must provide DAS with his social security number, email address, date of birth, home phone number and check the corresponding boxes for gender, race, education and veteran status. Trade: an industry for which the apprentice records the cost of the apprentice: all costs incurred by the apprentice must be covered. Expected completion date: The date the apprentice completes the program and determines a traveller`s status (if a credit is granted for previous experiences, it must be taken into account when filling the closing date) Agreements can be submitted apprenticeship@mass.gov. An apprenticeship contract must be signed at the beginning of the apprenticeship. It is used to validate individual employment agreements between the apprentice and the employer. Start date: the date on which the apprentice must have signed the contract and received the DAS within 30 days of that date. Recognition for previous experiences: credits can be granted, but no more than 50% of the duration of the apprenticeship. Credit experience may be granted by the program sponsor, but must be approved by the DAS Schooling authorization: when the apprentice signs the apprenticeship contract, he accepts the conditions on the back of the agreement and gives access to DAT to all associated teaching materials.

This agreement must be signed by the apprentice and the employer at the beginning of the apprenticeship. All apprenticeship contracts must be submitted by e-mail within 30 days of the start of the learning period and must be accompanied by school information, a passport image and an online payment worth $35 via the online payment system of the Laboratory Standards Department: www.mass.gov/service-details/department-of-labor-standards-online-payment. An apprenticeship contract is an obligation signed between the apprentice and the employer or sponsor. Previous employment rate: the level of salary received by the apprentice in a previous job. Number of hours per day/week/overtime rate: the apprentice works one day, more than one week, and what is the rate of overtime for hours worked during the normal work week. Signing the apprentice: the apprentice must sign the agreement which stipulates that they have read the terms of the agreement and that they accept. This document is a model provided by the ESFA to help employers. Wage scale: schedules and steps must be the same as what is established in the department of occupational safety, wage growth and percentages reflect the same employment in the region.

A wage change must be made when standards conflict. The first step should not be below the minimum wage as defined by M.G.L. Ch. 151 subsection 16. Contact the department at 617-626-5409 for current increases and percentages. Once all the information has been completed, it must be signed and forwarded to DAS for approval before it can be used as an exported document. Parent/guardian signature: legal guardian signature, if necessary. Training period: the length of time it takes to obtain the status of a traveller in a particular trade.
